Add Turn to GH Ticket features to the forum - Part 1

IntelliFactory created an issue 2015-04-03

We now have GH issues and standard forum entries in the WebSharper forums, and it is often necessary to manually elevate certain standard entries into GH issues. To save on this effort, we need a "Report on GitHub" checkbox when new forum entries are created. This would post the entry as a GitHub ticket using our own "WebSharper website" user or the appropriate GitHub user if we can match that by email.
