MakeDb does not support multiple D segment junctions

A some point we need to support multiple D segments in a single junction for TCR data. Right now, only D1-REGION is handled.

    v1.3 of the AIRR Standards will have Rearrangement fields for multiple D calls, so we can address this after that is released.
