Float Combination rules

Float the combination rules so that the section box is on the right. Make the label flow under the selection box or cut-off when it gets there.

    Rebuild combination rules so selection boxes are floated to the right. The text is floated to the left and clipped so it doesn't overflow. Changed fieldset insert so it is added as a sibling, not a child.
