"Create item" effect does not create item

I am trying to implement the spell Shadow Blade, by creating an item (a weapon named “Shadow Blade”), using the “Create item” effect, as shown:

However, when the spell is clicked and rolled, nothing happens. The character’s inventory does not have the Shadow Blade weapon. There are no errors in the browser logs.

Foundry version: 11.315

Obsidian version: 6.5.12
