Add support for using csurf_klinke_mcu in Visual Studio Community Edition 2019 and docs on how to get up and running.

Here’s a commit that I believe adds support for Visual Studio Community Edition 2019 and some docs that will hopefully help other developers.

I don’t know enough to know this isn’t going to break Visual Studio 2010 (which I believe you’re using from your comments on the forum) but I was hoping you might be able to provide that feedback.
