Spring '19 / API 45.0 updates

Update IC for Spring '19 / API 45.0 changes. I plan to release official builds of IC1 and IC2 with these changes Monday, February 11, after all orgs have been updated to the new version. As before, I will likely attach a pre-release build here for those who are already updated to Spring '19 earlier than that.

    Note that some Spring '19/v45.0 changes have already been delivered in the official build of IC2, in particular the ability to use API version 45.0 and support for LWC component development. I will provide a more complete update for both IC1 and IC2 (no LWC for IC1, though) no later than next Monday (February 11), but for those who have already been upgraded, you should be able to use IC2 against your Spring '19 orgs now.
