All the Local Only files are not on my filesystem - not sure how they’re getting there….

If you add the following to Help>Debug Log Settings and then click the Refresh button, it should provide a detailed log of how it builds that view:

#com.illuminatedcloud.intellij.builder.BuildOptionsDialog

ok, found that it was importing them from the IlluiminatedCloud directory for an old OST ConnectApi dir. I removed the OST and it’s resolved the issue.

Not sure how I got it added - I had manually set the directory to excluded - perhaps that’s it?

I'd need to see how your source roots were configured, but that certainly explains it. If you can get it back into that state, please send over your *.iml file(s) for review.
