Better new trigger creation dialog

Issue #7 resolved
Scott Wells repo owner created an issue

Need a dialog that takes three inputs:

  1. Trigger name - text field
  2. SObject type - selector
  3. DML events - checkboxes

and works just like standard template-based creation.
