Add a city report

Add a city report counting requests, pages, files, transfer and visits for each city reported by the GeoIP library.

the new report will follow the country report and the number of rows in the report will be controlled by the new TopCities configuration variable.

A separate tab-delimited file for cities will be generated if TSV output format is enabled.

Each city will have a city ID corresponding to a GeoName identifier from

http://www.geonames.org/

, which is what MaxMind DB reports in their Cities database.
