Mk2 IVA Visibility Improvement

The modifications to the Mk2 IVA and external model needed to make forwards visibility better are quite minor on the whole, but I decided to wait post 5.0, basically move the front kerbal up a bit to make his sigh-line clear a nose, raise the front part of the canopy to match, make the front of the canopy one or more windows instead of a metal wall.
