Grafting failed, joint too crowded

I get the above notification. However I am not sure why I get it. I have added a right arm + right hand. I try to add a left arm - which should be a different joint.
Just to make sure I did not confuse left and right, I tried adding a left and right arm several times.

adding a hand did the trick. I got confused because there was no indication that the arm was already added
