It seems that monster stepping on a teleportation trap do not actually teleport but vanish. At least if you step on the trap right afterwards you won`t find the monster at the target location.

Teleportation traps randomize the target location each time, so it’s likely you got teleported somewhere else than the monster.

As Krys said, the random target location is the current behavior of teleportation traps.
