Regrowth behaves oddly

I lost an arm and a leg and even kept the severed limbs in my bag. Used Regrowth on myself multiple times, but to no avail.

After moving to another level my limbs graphically reappeared even though they were gone. Later, the weapon that was in my arm that was severed reappeared in my worn equipment screen after i had used the Graft spell to reconnect my limbs. Now i graphically appear to have 3 arms and 3 legs but i really only have 2 of each. #1

.. and now i noticed that after moving to another level my appearance has normalized and i now show only 2 arms and 2 legs as usual. #2

i dont have this problem, maybe you forgot graft hands? when i grafted arm and hand together then in interface i have seen the slots
