Reconsider player to-hit and monster DV

Unlike in ADOM, you don’t get extra to-hit from levels. This means that your total to-hit is limited by items and a finite number of talents depending on weapon type. Certain late-game monsters (e.g. the priests on D:20) have amounts of DV that would be high even for ADOM. This makes melee suck.

    There was a bug in the level advancement code. Starting with the next release, to-hit probabilities also increase with level.
