My arm was cut off. I reattached it. However I could not equip my 4th weapon. I cannot add another hand onto the arms either.

Can you provide us with the saved game?

I kind of died in a dragon’s layer, so no. I fixed the issue via Spell: Regrowth before I went picking up the last stacks of gold my character saw.
