Chests occasionally respawn upon loading a save

I started noticing this on floor 3, and consistently on the higher, randomly generated levels. The problem occurred immediately after restarting due to a failed load on floor 4. The save I reloaded to had received the key. I've yet to be able to recreate the issue, but the chests all respawned a few tiles away from the ones I'd already looted.
