Symptoms

When the more the trigger set, the create issue is duplicated.

for example: 3 trigger set

1. The more the trigger set
2. Any trigger ,kicked off
3. three Issues created

Trigger is AND condition? is OR condition?

After Upgrade problem occurs. (Install varsion 4.0.2)
