Make an option to create pull request unapproved by default

1. Option to replace all automatic actions that clear "approved checkbox" with setting of "unapprove checbox". For example, "clear approved state on push" functionality should be affected. Also, if this option is enabled, default state of reviewer state should be "unapproved" instead of "not approved".

2. When creating pull requests, it will be nice to have checkbox:

I'm a little confused by what this is asking for. "unapproved" and "not approved" are not distinct states. The "unapprove"ing that this plugin does returns reviewers to the same state they already always start in; it does not put them in a "new" state. In other words, every pull request participant already starts out "unapproved", and they can toggle between "approved" and "unapproved" as many times as they wish.

I'm not sure what a "paused" pull request is. Can you elaborate?

Lacking any further clarification on what's being requested, I'm going to close this issue. As far as I can tell, the system already behaves in the desired fashion.
