Comments stop loading on large pull requests

When working with large pull requests, say of 100 files, comments stop loading for files near the end of the file list.

Correction, 280+ files.

Version 1.4.0 of the extension fixes this issue. Please re-open if you still see this happening. (Note that you need to update to VS Code 1.34 to use this version)
