move files pertinent to rv from init/ to cmd/rv/

There are files in init that are relevant to rv only. These should be moved to the cmd/rv folder. Further, there are some build targets in init/Makefile that are relevant to cmd/treatment. These should be moved into a Makefile dedicated to treatment and put under cmd/treatment.
