mts: Video clips should start with a PMT

Each MTS video clip generated should start with a PMT with a PSI. This should be true both for clips sent over the network, and clips written to a file.

NB: The PSI should be present even if metadata is not available, although as a principle it should always be available to the MTS encoder.
