broadcast: restart hardware for consecutive stream health errors

Occasionally stream health issues can be resolved by manually power cycling the streaming hardware. Now that we automatically monitor stream health, we can also power cycle automatically. If the stream health check for a given broadcast gets x (max poor healths) poor health stats in a give period, we should invoke a streaming hardware power cycle.
