<everyzig314> "Attack Ignored Hostile Creatures During Autoexplore" sometimes causes you to attack enemies when you move normally using the arrow keys (or numpad arrows). Furthermore, after moving adjacent to an enemy and attacking them once this way, if they survive and you move adjacent to them again (or move to another space that is also adjacent to them), this behavior can cause you to repeatedly attack the target until it dies. The latter behavior has happened to me specifically in the case of a sultan cult baetyl. I moved next to it with a numpad arrow, and it was autoattacked. I moved with a numpad arrow to another space next to it and my character repeatedly autoattacked it until it exploded.
