<TheBlackHand> Enemies spawn inside of walls, as is visible with clairvoyance. This is most marked on mountain map tiles. Attempting to move into one if you have an adjacent empty tile to approach one ejects them from the wall. This happens with things like campfires too, since I saw a little flame on one wall tile.

I have likewise encountered this issue, but this was an extreme case I had to report; dozens of Qudzu ‘misspawned.’
