Wanna take a look at the notes on this bug? With limb change it should just count as its own offhand.

Further testing by Jak:
[7:08 PM_]_its actually that if you have a folded carbide wristblade equipped and all of your hands are empty, the wristblade is basically treated as your primary and attacks 100%. if any of your hands have something in it though, the wristblade has that base 15% chance to attack, affected by jab and dual wield(edited)
[7:09 PM_]_i didnt test that extensively at all so you may wanna double check that but thats what i observed
I’d guess the game is taking it automatically as a primary limb ala horns without any weapons in hands.
