Followers falling down shafts in Bethesda Susa

In the beta, followers will randomly fall down the elevator shafts in Bethesda Susa during auto-explore. Quite rudely, the death is frequently attributed to the player (in one case a Lurking Beth, which was the last enemy to hit the follower).
