Unfortunately the only screenshot of it I have, I didn't think to take more when it happened, but I was somehow able to dismember a Wraithknight. I imagine this is probably unintentional, since the combat log stats that your attacks pass right through. I think it was specifically when I used Berserk into Flurry on him, with an axe in each hand. I was unable to decapitate him despite having the skill and hitting him a lot, but I did remove every other limb.
