<rockygr> <alpha> a Legendary Pariah NPC found on the stilt had a bunch of followers that were neutral to me. After the leader switched zones, the followers left behind became hostile and stopped following the leader.

Can be easily replicated using the save: just make the legendary npc switch zones by swapping places with them, the current followers that are neutral will become hostile as well.
