Quest item failed to spawn in designated ruin

The quest item "Holy Bottle" does not appear to have spawned in ruin designated by a village quest. No enemies appeared in the zone, I used no abilities that could have destroyed it, and I removed all the rubble in case it was hidden under it. Save file attached.

It looks like the quest item spawned without a chest for some reason (roughly SE of your position in the save). You must have auto-pickedup it without noticing.

only fetch quest items spawn in chests, this isn’t a fetch quest

Oh of course, it’s a “drink from” quest.
