Companions are still wielding armor and refuse to dual wield once their off-hand is full

I gave a companion a carbide dagger and a steel dagger, which they equipped. I later found another carbide dagger, but they didn’t replace the steel one. When I gave them carbide plate armor to replace their chain mail, they also replaced the steel dagger with the chain mail.
