I don't really know how to explain why this is the case from looking at
ShortBlades_Shank.cs
, and moreover I only have an ~20% understanding of how penetrations are calculated in the first place. But: to test this, I modded in a creature with a large health pool and 0 AV/DV and attacked it until it had several dozen status effects.Against this creature, my average regular melee hits get anywhere from 4-6 penetrations. In the first picture, this creature has 19 negative status effects, suggesting that shank should get at least 38 penetrations. However, shanking instead only gets 24 penetrations.
In the second picture, the creature has 14 negative status effects (all bleeds), so I should get at least 28 penetrations. Instead, shanking only gave 20.
In every case I tried, the numbers appear to be within such a range that (IIUC) I'm not getting a 2 penetrations per negative status effect, but rather, one penetration per status effect (on top of the regular number of penetrations from my hits).

This is because two penetration bonus is roughly equal to one penetration, and it’s doing the former. The description says
If you hit, the attack gets +2 penetration
so either it should do that or the description should specify it changes penetration value (PV). -
