    I was previously able to replicate this by wishing effect:Poisoned or effect:Ill, but I can’t seem to replicate it as easily by getting poisoned “naturally”, e.g. by a gallbeard or a snapjaw spearfiend. The first time I encountered this bug was in an actual game (unfortunately I don’t have the save anymore) so it seems like there are some conditions under which this can occur naturally, but I’m not sure what they are off-hand, sorry.
