Remove support for MTA and for very old PGI.
This commit follows-up on GASNet's removal of all support for the MTA (CPU, compiler and O/S) and for PGI compilers older than 7.2-5. MTA-specific code is removed and the minimum PGI version is documented in INSTALL.TXT.
