Public release of Draft 4

This issue is to track the release of Draft 4 for public review pursuant ratification.

The issues list still contains a few minor remaining issues slated for the 1.3 milestone,
but in the last telecon it was agreed we'd proceed with a public review draft and deal
with those and any other minor issues that arise in parallel, merging them into the
document before releasing the ratified final spec.

The updated draft is now available in our Downloads section, and if there are no objections,
this announcement will be sent to the world tomorrow.

I've added versions of Draft 4 to the download section that omit change annotations,
for a preview of formatting of the final document.
