Location of broadcast, barrier and allreduce

This is really a comment that I wanted to put on the source file, but couldn't find a way of doing it.

Why are the broadcast, barrier and allreduce operations in the teams chapter? Yes, they are collective operations on teams, but so are many other operations, e.g. atomic domain construction. So why select those particular operations and no others for inclusion in the teams chapter?

This issue was triaged at the 2018-06-13 Pagoda meeting and assigned a new milestone/priority.

In particular, we think it's important for collective operations to have a dedicated chapter, separate from the chapter on teams.
