Whither upcxx.hpp?

There is not upcxx.hpp in the source tree, but it is used in all coding examples. A good candidate s backend.hpp.

Until we fix resolve this issue none of the code examples while compile as is.

    This is a bug in the current implementation.

    We can have additional optional header files, but the spec requires <upcxx/upcxx.hpp> to exist.
