Parental Control Setup menu issues

The Parental Control Setup menu has a duplicated heading.

The last menu item is clipped (this is a direct impact of the duplicated heading at the top of the screen).

I have an XML fix for the problem but I made a mistake and lost the diff of the change. For the moment here is the XML code fragment that fixes the display issues.

Reproduction steps

From live TV press MENU > Setup > TV > Parental Control the screen should look like the attached image.
