publication page: add new document edit form after uploading new document

• initally, when list is empty, you try to add a document, the edit form does not appear. please fix it.
• please replace qr code button with preview button, but use for download button the qr download.

There is a link at the bottom of the document forms. If you click on it, you get the upload form.

On the /publication/HASH/USER page this works indeed. But on the postPublication page, it does not work: After the first document is uploaded, there's no "upload document" button any more. (BTW: I figured out that clicking on the "upload document" label opens the file upload dialog but this happened by chance and few users will find this)
