improve history page

The current history page shows differences between posts:

I suggest the following improvements:

1. The header of the page should be identical to the normal post page, i.e., http://history.biblicious.org/bibtex/28608f2aa76fae7b48ae2ca042d403c15/jaeschke such that the user name and the action buttons are visible (just include the corresponding TAGX file)
2. The mouse cursor when going over changed text currently is changed from an arrow to a hand (like over a link). It should become the same cursor as when going over text.
3. The underlining for newly entered text could be omitted.
4. Why are the "restore" buttons disabled?
5. The list showing the changes should be aligned such that the keys (e.g., "month", "tags") are right-aligned and the values left-aligned.
