Examples for ACSE & DIMSE usage

ACSE negotiation and DIMSE are a fairly complex subject: add more examples that explain how to negotiate the syntaxes and how to use them in commands.

Additionally, some command classes may be simplified (e.g. for commands that accepts only a limited number of abstract syntaxes they max have a reduced number of parameters in the constructor or they may check the used abstract syntax in the constructor).
