rst filename's NFC/NFD problem on Mac OS X

Sphinx 1.2b1 can handle the file name of the multi-byte string. However, there is a NFC/NFD problem on Mac OS X. Because of this, Sphinx can not recognize the file that is specified in toctree directive.

At Japanese environment, DAKUTEN cause this problem.

    In fact,

    1. Python open() method accept both NFC/NFD string and open same file.
    2. toctree's document entries were decoded into NFC.
    3. Python os.listdir(u'.') entries were decoded into NFD (like) on OS X.

    I need to fix 3rd behavior.
