Add global_toc to html builder environment

It would be nice to have the toctree for the master document in the environment for use by html themes.

With this, I wouldn't need a custom builder to drive my theme, and it might inspire some new innovative themes.

    Yes, that works great. Sorry, somehow missed that in the docs. And in the code.

    The wording of the documentation led me to expect that 'collapse' was defaulted to False. It was a bit of a surprise to find it was defaulted to True.
