[rst Domain] reference target conflicts with equally named directive and role

The rst domain generates the same reftarget for roles and directory with the same name. Examples of this bug can be found in the sphinx documentation itself, for instance .. py:class:: and :py:class: both link to the anchor #py:class.

Therefore, the documentation for the role :py:class: is not correctly cross-referenced, neither through a directive reference nor through the index.

To avoid this, the domain should encode the type into the reference target (e.g. #py:class-role and #py:class-directive.
