[patch] Exclamation marks included in output

After switching to 1.0 to build Psycopg docs, exclamation marks used to avoid link creation started appearing in the output. The docs use the default role (e.g. `!close()` configured to "obj")

The bug manifests when self.fix_parens is false: in this case the first character is not stripped.

The attached patch fixes the issue.
