HTML special characters not properly escaped in title element

I've seen two instances of this. If the title from a document contains something like '&', When this gets translated by the html builder, it is escaped in the <body>, but not in the <title> elements.

Similarly, if you leave out a title environment.py adds a title of '<no title>' to the page, which then gets added unescaped to the resulting <title> element of the built HTML page.

    I can't reproduce this. Can you tell me a bit more about the circumstances in which you observe this issue?

    In particular, what version of Sphinx are you using?
