Add a quick link for adding appointments to the Admin Tob Bar.

This is would very helpful on small screens, where the sidemenu isn't visible e.g. smartphones.

FWIW - I've never had a problem with the side-menu and/or sub-menu items not showing on a mobile browser on my phones and tablets. There might be a conflict with an additional plugin you are running, or possibly your mobile browser.
