reporter This has not been tested on WP 5.0 but on 4.9.8. here
But after updating to Appointment 4.2.6 it works again.

This is a Cron job issue, not related to the new update. My guess is that during this large update of WordPress, Appointments and WooCommerce some hosts don't assign enough memory for cron to schedule all tasks at once, so it gets stuck on "Now" for lots of events. Check you cron events with Crontrol plugin and simply delete all events that are have Next run stuck on "Now". It should work properly from there on.
