logout hide passcode by default

during logout, do not display the passcode, but create a button that will show the passcode when clicked.

work flow should be: user clicks logout link modal pops up with warning and button to show passcode, final button to log out is disabled 5 seconds later, final button to "log out" is enabled, effecting the logout if clicked
