Plugin cannot be launched in SSMS 2012

Issue related to #23. Launching SSMS got the error attached. The reason is that it's probably hard coded in the add-in's config file "Program Files" while at my machine the add-in was installed actually in "Program Files (x86)".

The path happened to be correct. The cause has some different roots. Will investigate.

1. There must be NLog.dll in SSMS2012 dll's folder. Can you please add it there?
2. Config does not work from this folder "C:\Users\maximn\Application Data\Roaming\Microsoft\MSEnvShared\Addins", but works from this one "C:\Users\maximn\AppData\Roaming\Microsoft\MSEnvShared\AddIns". Can you please change it as well?
