Try to install the add-in in SSMS 2012

I have added the instructions how to add the plugin manually in SSMS. If you have the solution built successfully, please try to follow the instructions to check if it works in your case.

I do not have SSMS2012 in place at all. I have windows XP and ssms 2012 could not be installed on windows xp... need to think what I should do.. probably I will add a virtual machine with windows 7
